De medaille werd ingevoerd op 9 juni 1945 en was bedoeld voor militair personeel die Warschau in Polen bevrijd hebben tijdens de Tweede Wereldoorlog.
Er werden 701.700 medailles uitgereikt gedurende en na de Tweede Wereldoorlog.
Aan de voorzijde staat: ЗА ОСВОБОЖДЕНИЕ oftewel Voor de bevrijding van Warschau.
